This is a patient that was almost ready to get restored on an immediate molar, but broke the one behind:
#14 was healing well, the patient came in and broke #15. This patient’s insurance was maxed out so we took an impression and planned for an Optiguide.
Very nice bone up in a sinus septum I angled so I could still be in the furcal bone of the molar:
You know the deal by now. Stabilize guide with a bite block. Drill through the tooth with the 2.3. Section and then extract the remaining roots.
There are a lot of drills leading up to a 7mm implant. This is the ID Guide Handle Kit

Tried something different. Rather than dealing with Bone Graft chips getting into the internal connection I placed the membrane through the healing abutment (I did not have my big ones so I used the freebie) and then lifted it up to place the graft and tucked in. Works better in theory than in practice as I had a hard time visualizing where my graft was going. Anyway we will wait 4 months to restore. We took a pan due to the fact it was difficult to get a good film way back there on the PA. You can see the gap around the healing abutment due to the membrane.
