Personally I would never attempt an immediate pre molar implant without a guide. There is a great amount of bone lingual to the mental foramen. The problem is that due to its superimposition over that bone, when you drill and place your implant it looks like you are right in the foramen! Scary! So by locking the trajectory of your drill with a guide you can have a little less “pucker factor”, while doing these. #29 fracture at the gumline, scan in CEREC 4.4 for the ssi and into Galileos and out as a CMG.DXD
While the guide was milling we extracted #29. Just love how big the blocks are. Placed seating window in the anterior.
Depth controlled with the head of the hand piece as a stop using my keys which you can get at the store at www.digitalenamel.com. Always like to get a film with my drill in.
I usually don’t take a post op CT scan, but look at how well the planned and placed images are! We grafted and placed a membrane over the implant. Such a smooth case!!
