Is It Hot In Here Or Is It Just Me? DEKKA laser assisted Guided Case

Here is one of those “Sounded like a good idea at the time” type cases that took a bit to complete.  Non restorable bridge #29-31.  Planned on an immediate molar implant in #31 and conventional #30

 

 

 

 

We did an upper immediate molar as well, but planned on an optiguide for the lower.  Just scanned the model with the bridge and Sicat sectioned it on their end. Really close to the nerve so we panned on a 3.7 by 8 for #30 and a 5.2 by 10 for #31 and plan on splinting them in the finals.

 

 

 

 

Planned on laying a flap and lateralizing some tissue from the distal.  Used my DEKKA CO2 laser, wow now thats got some power to it!

 

 

Got into it and just had no stability on #31, too much bone removal.  DFDBA and collagen membrane with primary closure.

Came back 4 months later.  Not in love on how the tissue healed on #30, so we planned an apically positioned flap, again using the DEKKA.  Pumped the tissue up with 1:50K epi as blood freaks me out ;}

 

 

 

 

Placed a Legacy 4 in #31.  Nice stability . Placed the flap apically and we will see how it heals.  #32 may need a bit of recontouring and will use custom titanium abutments to parallel things up.
