Here is an immediate case that I got to play around with the new Telio CAD blocks. Really cool! This patient had an abscess on #5, big ‘ol crown to deal with. Took a bio copy image and we are going to use that for the Telio CAD temp and also Saved As, to use as the SSI for the implant planning.
No proposal on the SSI just toggle the model off and on with those little eyes on the CAD/CAM menu. Due to the long 16mm implant and the angulation of the socket we had to angle the implant a little more buccal than I would like to get a screw retained restoration.
Use luxators whenever removing any teeth for an immediate implant. The more thin the better. Try not to “smooth” the papilla and keep your finger on the buccal plate when removing.
Insert the Guide. Went with an optiguide instead of CEREC guide just based on time when she came in. Always take a PA and LOOK at the angulation of the 2.3mm Pilot in the socket to make sure you are on the right track, at this point you can still recover if things were off.
In goes the Legacy 2. Again, if you are going custom, or just like screw retained crowns use the Legacy 2, if you know you are just going to use stock and a PFM, just use the Legacy 4. Both have the same body but different types of abutments. The 2 has a titanium temp, the 4 has a final with a nice platform switch.
I love the comfort of using a scan post after an immediate placement of an implant with some Gap distance. No acrylic touching the implant or getting stuck in the socket, which would make you lose your implant. Make sure that the scan post and the scan cap line up, there is a bit of “slop” so line them up and confirm that in your scan.
Horay for Telio CAD!! Need I say more, you can see my buccal placement and where my screw access is. No Bueno, but will still make it work with a temp, but the final will have to be a separate abutment and crown.
Use the Composite Binder with the Multi Link Abutment as well as the composite you place in the access. Filled up nice, no?
Had this pre milled as we are using the same file as before. Split it and did two separate RST files, one for a size 14 MO block and another for the Crown with a 12 block of Emax. Bulked it out a bit hence the blanching.
Having my on going bromance with Teflon Tape! If you aren’t using it what’s wrong with you? Torqued to 30, cemented with Multi Link. Love this stuff!! Yes, it does look funky as such a large tooth but thats what I was working with before. Look out for Telio coming soon!!
