Here was a consult that turned into a great easy Guided Case:
Pre-molar edentulous space, usually I can get a medium in there but it was tight so I went with a small.
Radiographic Guide on a Mach 2 model:
Good bone width. I’ll show in another image the crest to the sinus in the proper angulation would only give me 8mm of bone, but if I angled it a little to the distal (apex in the mesial) I had about 13mm of bone.
Always flip your Guide upside down to see if the Drill Body is seated. Try in the mouth and make sure it not only “clicks” in but does not rock.
Small Punch, lots of attached tissue.
2.3 then the 2.8mm drills. ID’s angulation pin has ends for each drill.
Placed with drills that torques out at 45Ncm, final indexing with ratchet. 4.2 by 11.5
Indexed with flat to the buccal.
Temp out of occlusion. Freebie abutment hand tightened with the 1.25mm screwdriver.
Planned and placed, looks like I am in the sinus, but I’m palatal to it.
Final, I usually like to do a fixture level impression, but I think this turned out acceptable.
