Here is another all digital, model and jig free case from Brighterway!! As I may have mentioned in previous posts, I have been volunteering at Brighterway which is a clinic that helps the homeless, low income vets and the working poor. They have 5 Trios scanners, a Carbon Printer and a Ceramill system from Amanngirbach. This patient was special to the clinic as he went from being homeless to working right there in the place that gave him back his smile!! We used Liquid Dam from Phillips to add “fiducial markers” to the gums to relate the scans. Big thank you to Dr Jonathan Abeniem for developing this technique. The most important step is recording the bite BEFORE you remove the hybrid and having enough of these markers present to relate the scans using the PRE PREPARATION option in Trios. DESS MUA level scan posts were used with a TRIOS scan.
The work in 3Shape Dental Desktop was done by the incredible Jacob Farber. We ran into a little angulation problem on #10 which Jacob covered with a crown!!
The hybrids were printed on the Carbon 3D Printer using Dentca composite resin and layered by the incredible Arian Deutsch using Anaxgum composites!!!
In this video I show the “Sheffield Test”. This is a test of how passive a hybrid is. The deal is you screw in one screw and push on the contralateral side. The hybrid should not rebound.
Here we are testing the bite. We did have some occlusal adjustment but nothing out of the ordinary for a full mouth rehab.
I posted on my FB page that I wanted to try letting the cement from the ti bases Harden on the mouth, was surprised by a lot of folks who said NO WAY! What I did was place max cement on the ti bases, wiped them off like the one on the right on the image and hand torqued in the mouth. Upon removal there was very minor flash. Used a sharpie to indicate where the flat goes.
Final all zirconia hybrid by Andrew Sedler at Burbank!! They did an amazing job, especially with the darker shade of gums and masking the opaque zirconia. I ended up not engaging #in 10 with a screw as we had 6 implants on the top, so its more of a “rest”. Will make things easier if we need to clean or repaid. Again, a huge thank you to all that helped with this case.
