We are pleased to announce a new contributor to Digital Enamel, Eddie Corrales! Eddie is the owner of CAD Smiles, a mobile dental lab that will come to your office and help you create beautiful restorations ranging from simple single units to full mouth rehabs! Eddie will be helping on this site with InLab as well as questions regarding CAD Smiles.
In this case Eddie went to the office of Dr Bob Marcus to help with a 4 unit anterior case. As with a lot of implants in the anterior, we get a bit of gingival recession. The question then becomes what to do about the gingival portion? Long teeth are ugly! With the advent of better gingival porcelain and stacking techniques its now possible to get a great esthetic outcome with gingival porcelain.
Although Scan Bodies can be used in the mouth, when dealing with an esthetically challenging case like this its not a bad idea to pour up an implant model with gingival mask. Also when using the Ti-base kits its very important to line up the notches on the tibase and the scan body. Not so easy in the mouth. The model and gingival mask were scanned in InLab 4.0
The case was designed in InLab and later the proposal was generated.
With sintering Zirconia and designing and stacking, Eddie knew this would take some time, so temps were made in Telio CAD on stock abutments as the patient’s restorations were fabricated. Ice was used to cool the water and speed up the milling process. Telio CAD comes in large blocks, but multiple single units can be fabricated.
Note the length of the proposals. Can’t do that with a stock abutment. Eddie separated the proposals into a crown and abutment, milled and sintered.
Emax value 2 crowns were milled and the gingival portion refined and stacked with gingival shade porcelains.
Final case in the mouth! Due to a low lip line you can barely see the ginigval porcelain, even though it looks great! The amazing part of this case was it was all done chair side! Nice work Eddie and Dr Marcus!
