Come On I Lean: #31 Optiguide Case with Mesial Tip and Custom Abutment

Here is a little simple finished case.  You know how I am always harping on about the curve of Spee?  If you don’t respect it on lower second molars the platform will be too distal and you will get a mesial cantilever.  Here is a finished case.  Note the Tangential view, we are leaning slightly to the medial and look at how “smoothy” the proposed CEREC is due to supra eruption, keep a mental note on the final.

Large Implant Direct guide sleeve uses spacers on the first two handles (first 4 drills).  Due to limited opening we had to “pre-load” the parts and pieces into the guide prior to rotating in the mouth.  Good trick before you abandon a case due to space restrictions.

Implant in and torqued to 45 NCM really dense D2 bone.  Contour healer cuz thats just how we roll.

3 months later, note the shape of the tissue and osseointegration.

Final crown with titanium custom abutment from Custom Direct by Implant Direct.  Note the shape of the crown and the limited occlusal dimension, predicted exactly on the CEREC Scan.  The lab was able to reproduce the emergence with the Contour Healer.  This to me is what Guided is all about, an easy cement appointment!
