I seem to be specializing these days in #29 Immediate with CEREC Guide 2! Another case that I wold never attempt free hand with the mental foramen where it is. Non restorable #29. Planned replacement with a 4.3 by 13 Interactive Implant by Implant Direct
You know the deal by now. Output from CEREC as an SSI into Galielos then after planning initially then out as a CMG DXD back to CEREC for further design. You can see that I like to keep my windows on the anterior. I added an extra window as my model had some distortion and I wanted it to seat.
Ok so here is a little complication. I use the Nobel Replace Tapered Guided kit as the medium sleeve fits really well. But the pilot I have with the kit is too short. So I use my IME keys for the first two drills (2.3/2.8) then finish up the rest with the Nobel drills. The guide was a bit thick so I needed the drill extender to get all the way down.
I have to say I am a big fan of fully guided insertion. Love the mount free implants and feel really good about having a stop and knowing where to index.
Grafted with DFDBA, collagen membrane. I am really loving these Villet reservable sutures from Implant Direct! Should restore in 6 months.
Ok here we are at about 6 months! Healed well. Normally I would have shaped the tissue but to be honest, we slipped on this one, so we did without. This is a 4.3mm Interactive so we are using the 5mm platform Nobel Active Ti Base.
So happy we have the 14 LT blocks now, just barely fit in it. Even with the negative 25 contacts were still tight. Surprisingly his tissue did not need contouring.
You all need to get Todd’s SRS bur. The white part removes the sprue rapidly while the blue part polishes. I like how it contours and removes any scratches. Since switching to spray glaze I don’t feel that the glaze really fills in the scratches like the paint on stuff.
After sintering I still like to go over the crown with Dashine and a Chamoise wheel.
Etched for 20 seconds, Silane for 60, then teflon tape, bonding agent, flowable. Torqued to 30 Ncm.
Sealed up added some stain for fun. Patient happy to get it all done in one day!
