Here was a quick fun case from today with no adjustments!! #8 impressive amount of erosion. Look at the PA vs the cross sectional, Damn! He’s one Dorito away from looking like a Carnival worker!
Out of CEREC Guide 2 blocks and they are all on back order. Didn’t want to mess with CEREC Guide 2 so I placed it free hand. You can see later on I wished I did it guided but will still turn out nice. Start with the lance bur at 45 degrees then straighten it out with the pilot. You can see that I am in line for an abutment but could have been more lingual for a screw retained crown.
I really butchered the tissue on my last anterior case so I was extra gingerly on this one with the papilla. It will be nice to see if they stay put. Slightly subcrestal, about 4mm below the future CEJ.
You guys know the cut tool trick right? Scan the gingival mask, copy it into the arch you want to scan for your restoration. Cut out the implant platform and then scan just the scan post. That way you can really get into the embrasures and contacts without the post in the way.
We can see that a more ideal position of the implant would have been more lingual. We will definitely go with a separate Emax abutment and crown. But for now with the Telio CAD temp we should be fine. Used Multi Link abutment and SR Connect to connect the frame to the ti base. (TSV 3.5)
Here is the Final. Yes I could have used a lighter composite in the access. In the mouth its not so noticeable.
Ok so 5 months have past and nw its time to restore. One of the many reasons I love Telio CAD is the fact that we make everything outside of the mouth and can really control the gingival contours of the temp without introducing material into the socket.
Due to the fact that the implant was maybe a little more facial than I would have liked we used a custom abutment and Empress CAD Multi crown. Look at the papilla on the gingival mask.
Here is the final. In retrospect I’m a little long. When the patient comes back we will shorten the incisal edge and take another photo. Cemented with multi link.
