Well I think everyone who knows me knows that I have mad love for Implant Direct. I have keys that adapt the Spectra system non guided drills for CEREC Guide 2 and have been a user for about 6 years. One thing that I have not loved is the fact that due to the fixture mounts that are pre attached to the implant I have not been able to place my implants through the guide (fully guided). Now that is not the end of the world, and have been doing pretty good placing my implants without the guide in place. Well, looky here! Implant Direct will be soon releasing their “Simply Integrated” line of mount free implants!!!! I got one to place with and I am really excited!! I have been meaning to switch to the InterActive line of implants which are surgically and prosthetically compatible with the Nobel Active Implant!! Ok, there is a bit of jerry rigging to get going on this so here goes. First, the Nobel Active Guided Kit, I have heard has removable stops and is a little cumbersome. Also the guided mounts are really skinny so they flop around in the CEREC Guide 2 sleeves. What I did was bought a Nobel Replace Tapered Guided kit off Ebay. The Guide mounts that come with it are for the Nobel Replace line which is a trip lobe so obviously it wont work, so I bought the NP, RP, and WP platform Guided mounts for the NOBEL REPLACE CONICAL line of implants. They have the same internal connection as the InterActive and Nobel Active implants but have the outer diameter that works with the guide sleeves of CEREC Guide 2.
Here are some shots of how this works. I found that the Medium Guide sleeve and the RP Guided mount works the best. Also the Nobel RP key works really nicely with the Medium sleeve as well. So you wont need to buy CEREC Guide keys for this. But you can see how the countless vial works and how the Replace Conical Guide mount interacts with the implant. The RP works with the 4.3 and 5mm InterActive implants as the both share the same internal connection.
Pictured is the RP key and drill in the Medium block. Note that these are CEREC Guide 1 blocks but the sleeves are the same for Cerec Guide 2. Now the NP sleeve works as well, but its a little loose. That being said I wouldn’t have a problem using it, but use as your own discretion. You could use my size small key with a 2.3mm pilot to start the osteotomy and finish with the 3.5mm Nobel drill and then use the mount. Unfortunately the WP mount does not work or the drills with the large guide sleeve. This is also true for the Nobel Replace guided kit and is nothing new. So in a nutshell, look for these mount free implants in the next few months from Implant Direct and I’ll post cases as I get them!!
