One Man’s Trash Is Another Man’s Come-Up: Legacy 1 Guided Case

Implants Direct is always coming up with cool new implants. We have the Legacy 4 with its large platform switch and aggressive threads.  Then we have the InTeractive, with its nice conical connection. So why, may you ask, am I using the lowly Legacy 1?  Well, I have to say, I have been getting a bit snobbish of late.  I love custom abutments, I just can’t stand to use a stock abutment anymore.  So the Legacy 1 is at a puny $150 while the Legacy 4 is at $220.  So why pay for what you don’t need?  Here we have a patient missing #18 and #19. Very good bone and a lot of room.  So we Tx planned for an optiguide with #18 and #19 5.2 by 10s using the large guide sleeve.

 

 

 

Scanned a stone model and incorporated the .ssi file.  Again, lots of bone but the nerve is palatal to where I am heading, so don’t freak out when you see the post op image.  Normally I don’t like to free hand CEREC ssi with distal end cases, but I did not have time to make a radiographic guide is Barium Sulfate teeth, so I took a buccal bite off an upper and lower model and made due.  But again, when you are doing a free end case, is tough to get the spacing right with just CEREC.  But I made it work.

 

 

 

 

I will release a prototype of my CEREC Guide/Sirona punches soon. I am not in love with the final design, but if anyone wants them, I’ll sell the prototypes to you and give you the real deal later.  Big time limited opening!!!  Cant even get the bite block in the right way!!  And whenever you are using an optiguide you either have to use a bite block or an Isolite.  So in the limited opening cases, “pre -load” your drill and handle and gain a little bit of length.  Still it was tough!

 

 

 

 

 

Used the head of the hand piece as the stop.  D2=22mm  Nice and easy, D2 isn bone.

 

 

 

So again, the Legacy 1 has no abutment.  Which is cool as you don’t need anything if you are doing custom.  No abutment screw either BTW, which sucks as I hate the CEREC Ti-base screw, but I have so many laying around in my drawer I don’t care.  Very nice torque values and finished up with contour healers.  The two implants were slightly closer together, so the distal contour healer is a bit rotated, but I am sure the end result will be fine.
