Ain’t Nobody Got Time For That! Immediate Upper Molar Implant

I dislocated my right shoulder falling down running last Wednesday. After being put out as the ER doc snapped it back into place I ended up having to cancel my day Thursday.  BTW after having tried Vicoden for the first time I can say it is rather a rather wimpy pain med!  But back to the office on Monday and its even more of a free for all than normal.  I was welcomed to the office with two really tough endo’s a few crowns and then this in the afternoon.  Great patient taking care of a sick spouse and no time for multiple appointments, so lets get ‘er done!  #3 fracture lots of decay and silver point endo.  Ferrule on the buccal but really nothing on the palatal. In the past I would have tried to re treat the endo, get a post in there and pray it lasted 5 years, no more.

Squeezed this implant in between two other crown preps, so no time for the cerec guide.  Flattened the root, drilled through the furca with a surgical 557, then sectioned the roots.  I had heard of a cool technique where instead of drilling out the furcal bone, you use expanders.  I needed about a 3mm lift anyway, so I gave it a go.

I have to say that I love it!  We know there are two ways to create an osteotomy, you can drill it or expand it.  Used the MIS expanders to compress the furcal bone up to the 4mm then used the crestal bone drill a little to make room in the socket.  Notice the furcal bone on the buccal and the palatal is squeezed into the MB root socket.

I dont know how I ordered a non HA Legacy 2 7mm, but here it is.  Excellent stability!

Particulate, membrane on a healing abutment,  yada yada.  It looks like I am close to the palatal root of #3 but you can see from the occlusal that I am buccal.
